Containment first. Then the filling comes out.
The IAOMT SMART protocol wraps the drilling moment in isolation, suction, aerosol capture, cooling and filtration — a coordinated system, not a single device.

Virginia Biological Dentistry is out of network with dental insurance. We have hundreds of patients with dental insurance who come to see our biological dentists and pay fully for services rendered. As a courtesy, we file with their dental insurance after the treatment. The dental insurance sends reimbursement checks to patients directly for the amount determined by their coverage.
Rather than a single device, it is a set of coordinated practices that together govern how an amalgam is removed:
Each item addresses a specific exposure pathway, and the standard expects them to be used together rather than picked individually. Dr. Olivia Hart has followed the IAOMT's SMART protocol since 2015 — and beyond it — refining safe removal of leaky and damaged amalgam restorations over many years.

Request ConsultationNo single barrier catches everything. Particles, aerosol and vapour each travel differently — so suction, isolation, capture and filtration cover pathways any one of them alone would miss.
The filling is cut into larger chunks and lifted out whole instead of being ground into dust — less becomes fine particle and aerosol in the first place.
BPA-free composite, ceramic inlays, onlays and overlays — with biocompatibility testing where indicated.
Dr. Olivia Hart is a Certified Biological Dentist and PROTECT Protocol Provider for safe mercury removal with the International Academy of Biological Dentistry and Medicine (IABDM). She is an Accredited Member of the International Academy of Oral Medicine and Toxicology and IAOMT SMART certified since 2015. Equipment is only half of the protocol — the other half is the trained judgment to run it consistently: positioning suction and capture correctly for each tooth, sequencing every protective step in the right order, and verifying each layer is active before drilling starts.
Preparation starts before the handpiece ever touches the filling.
Each restoration is assessed individually — condition of the margins, your health history, detoxification capacity, sensitivities. That profile shapes which fillings warrant replacement and in what order.
Air filtration is running, suction and external aerosol capture are positioned for the specific tooth, protective coverings are arranged. Every system is active the moment drilling begins — not adjusted mid-procedure.
A dental dam separates the treated tooth from the rest of the mouth, so fragments and slurry stay off surrounding tissue and away from being swallowed. It also gives a clean, defined working field.
The amalgam is cut into larger pieces and lifted out whole rather than ground away. Continuous water cooling carries heat off the bur, limiting how much vapour is driven off while the filling is cut.
The tooth is restored with BPA-free composite or a ceramic inlay, onlay or overlay. Filtration keeps running afterward and the working area gets a defined cleanup — a required step, not an afterthought.
